Application Checklist
Please be sure you have completed all of the following items:
[ ] Requested Official College Transcript (s) from every college/university attended and have them
forwarded directly to the Physician Assistant (PA) Program AND the University Application
Processing Center (UAPC).
[ ] Included a personal statement with the application.
[ ] Attached a 2"x2" photo of yourself to the PA Program application.
[ ] Completed "Reference Release Form" with the names and addresses of the three (3)
references entered on your application. Enclose this form with the completed application.
[ ] 1. Download and Print the “Reference Instructions” & “Reference Questionnaires”. Have
your reference (noted on the Reference Release Form) complete the reference questionnaire.
NOTE: All reference questionnaires must be accompanied by a recommendation letter on the
official letterhead/stationary of the individual providing the reference. See next item.
2. Requested THREE (3) Letters of recommendation on official letterhead/stationary to be
attached to the "Reference Questionnaire". See above item.
3. Include the THREE (3) completed references, in their signed and sealed envelopes with your
completed PA Program application.
[ ] Sign and date your PA Program Application.
[ ] Completed the CUNY transfer application online before the February 1st deadline.
