Instructions for TB clearance

New Volunteers (onboarding clearance):

You will need health clearance (TB) from Employee Health Services (EHS). Once you have had your photo taken

by Human Resources, please bring the yellow carbon copy receipt to EHS to begin the health clearance process.

Annual Clearance Instructions:

Please take your badge to EHS to obtain your annual health clearance.

Employee Health Details:

Employee Health Services is located in the Faculty Medical Office (FMO) at:

11370 Anderson Street, Loma Linda, CA 92354, Room 1250.

The FMO building is on the south west corner of Barton Road and Anderson Street across from the Medical

Center and Children's Hospital.

Hours of Operation:

EHS hours are Monday through Friday from 7:00 a.m. to 5:00 p.m.

o Walk-Ins welcome from 7:00 a.m. – 11:15 a.m.

o Appointments required from 12:30 p.m. – 5:00 p.m.

 Please call 909-558-8797 to schedule.

o EHS is closed for lunch from 11:15 am – 12:30 pm

 Closed weekends and holidays.

 TB testing is not available on Thursdays.

Additional Information:

 Teens must bring a signed parental consent form.

 If a chest x-ray is required, please contact EHS for specific instructions at 909-558-8797.
